The OC coordinates with participants, support workers, and stakeholders to deliver person-centered, culturally competent, and recovery-focused services. This role acts as the primary link between the service delivery team and the Operations Lead.<\/p>\n\n\n\n<p>Responsibilities:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Develop NDIS service agreements and schedule supports in line with participants&#8217; funding and needs.<\/li>\n\n\n\n<li>Communicate effectively with participants and stakeholders to identify goals and support needs.<\/li>\n\n\n\n<li>Provide relevant information to promote participant choice and control.<\/li>\n\n\n\n<li>Coach and guide teams to ensure dignified, community-based support.<\/li>\n\n\n\n<li>Adhere to privacy and data recording requirements.<\/li>\n\n\n\n<li>Manage and maintain accurate records and client information.<\/li>\n\n\n\n<li>Stay informed on sector developments and internal service pathways.<\/li>\n\n\n\n<li>Collaborate with teams to identify and address service barriers.<\/li>\n\n\n\n<li>Produce performance reports to monitor progress and identify challenges.<\/li>\n\n\n\n<li>Foster a culture of safety and continuous improvement.<\/li>\n\n\n\n<li>Prepare and submit reports as required.<\/li>\n\n\n\n<li>Liaise with Support Coordinators to ensure seamless service delivery.<\/li>\n\n\n\n<li>Attend industry events to stay updated and build relationships.<\/li>\n\n\n\n<li>Draft service plans and monitor client progress.<\/li>\n<\/ul>\n\n\n\n<p>Pre-Requisites:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Diploma in Community Services or a related field or equivalent.<\/li>\n\n\n\n<li>NDIS worker screening check or willingness to apply.<\/li>\n\n\n\n<li>COVID-19 Vaccination Certificate.<\/li>\n\n\n\n<li>Smartphone for app use.<\/li>\n\n\n\n<li>Current first aid\/CPR certificate or willingness to attain.<\/li>\n\n\n\n<li>WA Class C Driver\u2019s License and fully insured vehicle.<\/li>\n\n\n\n<li>Experience in individualised assessments and procedural change.<\/li>\n\n\n\n<li>Strong interpersonal, communication, and negotiation abilities.<\/li>\n\n\n\n<li>Experience in coordinating services within NDIS plans or similar.<\/li>\n\n\n\n<li>Excellent critical thinking, time management, and organisational skills.<\/li>\n\n\n\n<li>Proficiency in MS Office and basic computer programs.<\/li>\n\n\n\n<li>Ability to build rapport with stakeholders and work collaboratively.<\/li>\n\n\n\n<li>Problem-solving and project management capabilities.<\/li>\n\n\n\n<li>Experience in training staff on organizational and NDIS requirements<\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>Starting Mid October, 2024.
